Depending on how your web browser is configured, you may be able to print the "paper" form directly from your browser. Otherwise download the PDF file to your computer, open the file, and print it (requires a program that can open a PDF file such as the free Reader from Adobe). The file is also a typewriter-enabled pdf file and can be completed using the free Reader program from Adobe. Both mail and fax registrations will be acknowledged by email.
