Pre –Registration and payment deadline is April 2, 2012. Registrations postmarked after this date will incur a $25 late fee.
Fees:
Registration includes access to sessions, exhibits and CEUs as well as the wine and cheese reception, refreshment breaks and closing brunch. All attendees and presenters are required to register and pay the appropriate fee. You will receive a confirmation letter once registration is processed. If paying by Purchase Order, the PO form must accompany your registration (PO numbers are not accepted).
- $200 Member Fee (NTRS, ATRA, State TR Association)
- $200 Member Fee (Other Health Related Professional Association – association name required on form)
- $215 Non-member Fee
- $170 Student Fee (currently enrolled undergraduates – photo student ID is required)
- $145 Single Day Fee (Monday, Tuesday or Wednesday)
- $75 Student Room Host Fee (prior approval required: send form, photo ID and letter from advisor)
